☞ 1. a) NAU School of Communication provides the use of School owned equipment to students for specific School of Communication classes according to priorities and allocation of resources established by the school’s faculty in charge of the equipment. Some equipment is dedicated for certain courses and therefore may not be available for use by the students and faculty if they’re not enrolled in those specific courses. During the semester, students taking production courses have priority for certain equipment. Once priority has been established, requests are handled on a first-come, first-serve basis. b) Students have priority over faculty on checking out equipment for specific courses they’re enrolled in.
☞ 2. a) Once the students or faculty have taken possession of the equipment, they are solely responsible for all damaged and missing equipment. Any broken or missing equipment will be charged for replacement fee ranging from $10 to the full replacement cost. This may result in holds placed on the student’s account until the bill is paid. b) All prices listed on this form are estimates. Final costs, including tax and shipping, will be determined by the School of Communication. c) The students and faculty will pay for either the repair or replacement of broken equipment and materials. The School of Communication will determine whether a broken piece of equipment will be sent out for repair or require full replacement. d) Any stolen equipment must have an official police report & full replacement fee applied to the owner's account.
☞ 3. Students and faculty agree to return equipment as scheduled, in clean and proper working order (including charging used batteries for video cameras). Dirty equipment will be assessed as a $35 cleaning fee at the determination of the School of Communication.
☞ 4. a) Equipment returned late will be assessed a $35/day charge. If students and faculty are unable to make the return time/date for equipment, they must contact the checkout room BEFORE the time/date the equipment is due. Students and Faculty are allowed 1 renewal on their reservation. (An extra 24 hours) b) A second late offense for lateness and/or broken equipment will result in checkout privileges being revoked. c) Equipment overdue more than ten days will be reported to law enforcement authorities as stolen property and a full charge applied to the student’s NAU account. d) If students need equipment longer than the standard checkout, permission must be granted by the faculty in charge (in writing) and by the person running the check-out facility (who determines reservations).
☞ 5. Students and faculty are responsible for verifying the existence and operating condition of all equipment being checked out. Report any problems or missing items before leaving the checkout area.
☞ 6. Students and faculty must report any known damage or equipment problems when checking in equipment. Students and faculty are responsible for the equipment until it is all checked in.
☞ 7. If students and faculty fail to pick up any reserved equipment by the specified date and time, they forfeit the requested equipment (unless no other students or faculty are waiting for the equipment).
☞ 8. Students and faculty may not loan, lease, or rent NAU School of Communication video equipment to other parties, except other current students listed on the Team. Other students may return the equipment under your name. But be aware that if anything is missing or broken upon return - you are still responsible for the fee.
☞ 9. Equipment cannot be taken out of the state or country without written permission from the faculty of record.
